Animation & Illustration Student Safety Information, Quizzes and Forms

All students enrolled in Animation & Illustration classes in the School of Art must complete the steps outlined below in order to use the facilities and equipment provided in the Classroom and Studio Spaces.

If you plan on utilizing Extended Hours in School of Art Classrooms or Studio Facilities outside of class time and outside of scheduled supervised times, the following steps must be completed before application review:

  1. Please fill out and sign the   (If you have already completed the Safety Instruction Verification and Agreement form, go straight to your quiz below!) 
  2. After reviewing the Animation & Illustration Health Safety Information (link to the document is above), please check and confirm your understanding of the information with the
    • You MUST receive a score of 100% on the Safety Quiz
    • IF your score is lower than 100%, please review your submitted quiz answers, look over the information once again to learn the answers to the question(s) you missed and again.  Resubmit your answers and continue repeating this step until you have received a score of 100%.
    • There are no penalties for retaking the quiz.
  3. After receiving a 100% test score on your Safety Quiz and submitting the Safety Instruction Verification and Agreement Form, please fill out the for the program you're applying for.

After review of your required forms and receiving 100% on the safety quiz, an approval email will be generated for your Extended Hours Authorization Access.
 

N95 Dust Mask 

If you believe you may use a disposable N95 Dust mask or know you will wear one, go through the before use. This waiver is required every semester and applies to all your enrolled courses in which an N95 is used.
